Course Objectives
NUR 453: Chronic Care
Upon completion of this course, the student is able to:
  1. develop a body of knowledge that will form a holistic framework for managing the care of clients with chronic alterations in wellness.
  2. describe chronic illness/disability problems and issues.
  3. conduct and document a holistic health assessment of an individual family with chronic alterations in wellness.
  4. formulate and document-nursing diagnoses based on client assessment data.
  5. develop and implement a culturally congruent holistic care plan that incorporates practice ready research.
  6. describe the case management process and the role of the case manager.
  7. describe the ethical, legal, and economical aspects of case management.
  8. collaborate and communicate effectively with chronically ill-disabled clients, their families, and health professionals.
  9. act as an advocate for clients with chronic alterations in wellness.
  10. explain one's own feelings, attitudes, and behaviors and their effect on nurse-client interactions.
  11. develop a community resource directory appropriate for clients with chronic alterations in wellness.
  12. initiate appropriate referrals to community resources for clients with chronic illness/disability and their families.
  13. describe the characteristics and responsiveness of a community agency designed to serve clients with chronic alterations in wellness.
  14. discuss ethical and legal issues associated with selected chronic patterns of altered wellness.
